In this session, you will learn about Palliative Care Community Teams (PCCT). A PCCT works with community partners and healthcare organizations support individuals and their families through times of illness, death and bereavement according to their needs and wishes.

You will hear from PCCT members from Community Care City of Kawartha Lakes (CCCKL) and Haliburton Highlands Health Services (HHHS).

Who should attend?

Everyone is welcome to attend, including health service providers, primary care and community nurses, pharmacists, general community members, and more.
