Social prescribing is an evidence-informed approach that connects individuals to non-medical, community-based programs and supports to improve health, wellbeing, and social inclusion. Rather than relying solely on clinical interventions, social prescribing recognizes that health outcomes are influenced by social, emotional, and lifestyle factors. Social prescriptions can include a wide range of activities tailored to an individual’s needs, such as cooking groups, art classes, diners’ clubs, friendly visiting, and exercise programs.
Through this approach, social prescribing enables older adults and vulnerable community members to build meaningful social connections, increase physical activity, improve confidence in self-management, and maintain independence, while strengthening the integration between primary care, community organizations, and health system initiatives.

The Wellness Connections Program is designed for people aged 65+ to learn about healthy changes and choices to age well and to connect to local service providers offering supportive programs. The program can be accessed virtually through our website or with in-person support.
Each week we invite community to drop in to our space to socialize with others, learn new skills, meet new people and get connected with community supports. Located at 21 Angeline St. N in Lindsay, we offer social activities, guest speakers, exercise classes, cooking demonstrations and so much more!
